Step 1: Complete the Preliminary Application and Waiver
In order to schedule an appointment, download and complete the preliminary application form and waiver (at right), and email the completed PDF to firstname.lastname@example.org.
It’s recommended that you access this page using Google Chrome, download/save the PDF form to your computer or network drive, and then open it with Adobe Reader (free download) to fill it out. We do not recommend using any web browser to open the form and fill it out directly on the web as most have issues with fillable PDF forms and the data is lost when sending via email. If this occurs, we'll request you complete the form again.
MAC users: Do not use the preview program pre-installed on your device to fill in the PDF form. Adobe Reader (free download) is the only program that will allow you to work with the form properly.

Step 2: Schedule an Initial Appointment
After submission of the Preliminary Application and Waiver, contact firstname.lastname@example.org to schedule an appointment with Karen Wachsmuth. Please indicate days and times that you are available to meet in your follow-up email.
30 minute slots are available Monday, Tuesday, and Thursday at the following times (please contact us directly if you cannot make any of these times and we will try our best to accommodate your schedule):
- 10:00-10:30 a.m.
- 10:30-11:00 a.m.
- 11:00-11:30 a.m.
- 1:30-2:00 p.m.
- 2:00-2:30 p.m.
- 2:30-3:00 p.m.
Step 3: Schedule a Follow-up Appointment
Schedule a follow-up appointment (if you have already filled out a Preliminary Application Form this academic year).
Follow-up appointments may be scheduled through email@example.com. Please be patient while we process your request; if you do not hear back within 3-4 days, please get in touch again.
