Optional Practical Training Reporting

Students on OPT are legally required to report certain information to OISS. Changes to any of the following must be reported within 10 days of the event:

  • Name, email address, or residential/mailing address
  • Employment: report your employer name, address, and supervisor’s email within 10 days of any changes or new employment
  • Immigration status
  • If you will leave your OPT employment before your OPT end date

This form is to be used by F-1 students/past students who are currently participating in OPT and still on an I-20 issued by the University of Iowa.  All information is required unless otherwise specified.

Multiple Employers

If you work for more than one employer, you must list the contact information for ALL employers.  If you have more than two, please send a separate email to ip-sevis@uiowa.edu to indicate additional employer information.

Self-Employed Business Owners

If you are a self-employed business owner, please write “self-employed” in the Employer Name field and give the business address you use. Indicate the date you went into business in the Employment Start Date field.

Performing Artists with Multiple Gigs

If you are a performing artist and work for multiple short-term employers (gigs), write "multiple gigs" in the Employer Name field.  You will also need to send OISS a list of gigs with employer information and employment dates on a regular basis by emailing ip-sevis@uiowa.edu. If you are employed by an agency, provide the agency’s name/address information.

Work for Hire (Contract Work)

If your work is based on a contractual relationship rather than an employment relationship, write "work for hire" in the Supervisor's Name field. Provide contracting company name and address as well as the start date and end date for the contract.
